Scheduling Your Appointments

In order to provide you with the highest quality of care, we like to schedule consistent follow up appointments with all of our patients. Since all our patients require a variety of services, we will schedule your next visit according to your personal needs. Please consider the information below when you are making your next follow up visit:

SAME DAY APPOINTMENTS

Most days an appointment can be scheduled the same day.  Please contact the office and see what is available.  The great thing about the same day appointments is that this does include NEW patient appointments as well.

 

BLOOD DRAWS

Sometimes, Dr. Pukis will request you have blood drawn before your next visit. This should be done about one week prior to the follow up visit. In order to schedule your blood draw, please contact our office to set up an appointment between 8:00 - 9:00 AM.

NARCOTICS - 1 MONTH

If you wish to have refills on your controlled medications (such as narcotics, pain medications, etc.), then please schedule your next visit with us exactly 30 days away from your previous visit. If the appointment is scheduled for less than one month after your previous visit, Dr. Pukis will not be able to refill your controlled medications.

ANXIETY - 1 TO 3 MONTH

If you are needing a refill on your anxiety medication, please schedule your next visit 3 months away from your previous visit. Appointments scheduled too soon may result in Dr. Pukis not being able to refill your prescription.

DIABETIC - 3 MONTHS

If you are needed a refill on your diabetic medication or supplies, please schedule your next visit with us 3 months away from your previous visit. This will allow us to monitor your diabetic needs closely and consistently.

HYPERTENSION - 6 MONTHS

If you are seen regarding your hypertension (high blood pressure), we request that you make your follow up visit 6 months away from your previous visit.

WELLNESS VISITS - ONCE A YEAR

Most insurances pay for their members to be screened once a year as a general wellness visit. This appointment includes bloodwork, as well as an overview of your general health. These appointments do require that you fill out our paperwork prior to the visit, so make sure you have your packet before you leave your previous visit. Each insurance company does have different guidelines for scheduling, so please contact us for information if you have questions. 

 

If you have any other complications between visits, you are welcome to call our offices and schedule an additional appointment. These guidelines to scheduling your appointments are for your benefit!
