Are you accepting new patients?

Yes! If you would like to be established with our practice, please call our office today and we will make an appointment for you that best fits your schedule.

PLEASE NOTE: If you are a new patient and are needing a refill on controlled medications (such as narcotics or anxiety medications), it is up to the provider's discretion as to whether or not those controlled medications will be filled on the first visit. We may provide refills on follow up visits, without providing them on the first visit.

I want to be a new patient; do I need to fill out paperwork?

The new patient packet that needs to be filled out for your first visit can be found under the "Forms" tab located at the top of the page. If you do not have the ability to print the forms, please arrive in the office about 30 minutes prior to your appointment time to fill the forms out in the office.

What should I bring to my appointment?

Please be ready to provide us with information such as your name, number, current address, social security number, and current insurance. We also ask that you bring in bottles of any current medications you are using or need to refill.

If you are a new patient, we ask that you bring a photo ID and your insurance cards so we can save them on file for you.

How much is a self-pay appointment?

If you are a self-paying patient, the initial visit to be established with our practice is $105 and all follow up visits are $75. We are unable to bill self-pay patients for appointments, so please have your payment with you when you arrive for your appointment.

Can I have my blood drawn and tested in your office?

We do in-office blood draws Monday - Friday between 8:00 - 9:00 AM. Please call our office to schedule an appointment to have your blood drawn during these hours.

Can I take my medicine before I come in for a blood test?

Yes, you may take your medicine before coming to get your blood drawn as long as the medication does not require that you eat something with it, and as long as the medication does not alter chemical levels in your blood stream (such as thyroid medication). If you have questions about whether or not you can take your medicine, please call our office.

How long does it take for my blood results to come back?

It usually takes about a week before we recieve the results from the blood tests drawn in our office. As soon as we have had a chance to review the results, we will call you to discuss our next steps.

I was referred to another doctor at my last appointment. How long will the referral take?

Because we receive several referral requests a day, and each referral requires several different forms filled out, a referral to another doctor's office can take a week or longer. Please understand that we are working on the referral as fast as we can. Once the referral has been made, you will receive a call from either our office or the doctor's office that you were referred to with an appointment time and date.

Can I call in to get my medication refilled?

We no longer accept medication refill requests outside of an office visit. This is to make sure we are still giving you the best possible dosage and strength of the medication you are taking. We understand this is an inconvience for some people, and apologize; we feel this will allow us to provide the best possible care for all our patients.

What if I need my controlled medication refilled?

Because of FDA restrictions, we are only able to refill controlled medications after a certain number of days have passed from your current prescription. Because of this, controlled medications cannot be filled if you come in too early. If you are unsure if your medication is considered a controlled substance, you may call our office or click on this list of controlled medications.
