On your first visit to our office, you will need to provide important information such as your phone number, address, current insurance carrier (please bring any insurance cards with you), social security number and birth date. When you become a patient at Blossomwood Medical, a comprehensive medical chart will be started for you which contains a continuous record of all the care you will receive here. All information in your medical record is strictly confidential and will not be released without your written consent.
